Patient Home Navigator Certificate

Eastern Gateway Community College’s program in Patient Home Navigator offers students a one-year accelerated certificate program. Upon completion of the one-year certificate patient home navigators can serve in both community and healthcare settings.

Patient Home Navigators help patients receive the best healthcare possible by helping patients and their families navigate the healthcare system that is both complex and daunting. They navigate patients and their families through diagnosis, treatment, prognosis, and often into survivorship or end of life.

Patient Home Navigators perform many duties including:

  • Assisting patients with insurance issues
  • Scheduling appointments
  • Arranging inpatient and outpatient hospital admissions
  • Assisting patients with advance directives
  • Identify community resources available to patients
  • Lead patients to preventative medical screenings
  • Provide health information and education
  • Identify and reduce barriers to healthcare
  • Find financial assistance
  • Arrange transportation
  • Arrange home healthcare/hospice
  • Coordinate Durable Medical Equipment needs
